INSTRUCTIONS FOR PAYING BY VENMO

1. Complete the form, enter your VENMO ID below and hit the submit button (after reviewing these instructions).

2. Go to your Venmo app and connect to Venmo ID: @LVP-Treasurer.

3. Send the payment amount of $225 via Venmo to the Venmo ID above. 

4. If Venmo asks you for a confirmation code you may enter digits 6167.

5. You will receive a Thank you pop up after hitting the submit button. The thank you message will contain the LVPCA Venmo ID and confirmation code.  An email with a summary of the jotform submission from the LaVista Park Civic Association will be sent to the email address specified on the jotform.

6. The sign up process is not complete until both the jotform are submitted, and the payment via Venmo has been received.

INSTRUCTIONS FOR PAYING BY ZELLE

1. Complete the form and hit the submit button (after reviewing these instructions).

2. Go to your Zelle account and add treasurer@lavistapark.org to your Zelle recipients.

3. Send payment amount of $225 via Zelle to the LVP recipient above (treasurer@lavistapark.org). 

4. When entering the payment, please specify your LaVista Park property address in the Memo box of the Zelle payment. Failure to provide property address will delay your membership onboarding. 

5. You will receive a Thank you pop up after hitting the submit button. The thank you message will contain the LVPCA Zelle ID (treasurer@lavistapark.org).  An email with a summary of the jotform submission from the LaVista Park Civic Association will be sent to the email address specified on the jotform, and an email from your bank summarizing the Zelle transaction.

6. The sign up process is not complete until both the jotform are submitted, and the payment via Zelle has been received.

The cost for LVPCA membership is $225. There are two options to submit the check: 

a) Drop in the secure mailbox on the Brookforest side of the LaVista Park (this is          preferred)   OR 

b) Mail your check to the following address:
