ALFT Post-Project Feedback Form

The purpose of this form is to get an overview of how your project/workshop went. We’d like to know what went well, and we’d also like to know any issues or difficulties that arose so you can improve for the future. Due within one week of completion of your project and no later than March 2, 2026.

1. How did the Project go overall? What is your overall assessment and feeling about it?

2. Did your project meet the parameters you originally described in your Project Description Form (area of focus, number of participants, contact hours, etc)? Describe any deviations from your original PDF.

3. What went really well? Where did you personally shine? What do you see your gifts are from this experience?

4. Were there any unusual issues with participants (unusually strong resistance, coming late, someone leaving, concerning trauma vortexes and/or psychological breakdowns, etc) that you had to tend to? If so, please explain how you handled that.
