    I, the undersigned, hereby acknowledge and understand that by participating in the “Relax, Reset, & Renew Women’s Empowerment Retreat”, organized by Mia Bolden. I am voluntarily participating in activities that may include, but are not limited to, yoga, meditation, hiking, wellness workshops, group discussions, team-building exercises, and other recreational activities ("Activities"). I understand and acknowledge that some of these Activities may involve physical exertion, exposure to outdoor conditions, or emotional vulnerability, and I accept the risks associated with such Activities.

    I understand the following:

    The Activities may present inherent risks, including, but not limited to, injury, emotional distress, or medical emergencies.

    I am fully responsible for my own safety, well-being, and actions during the retreat, and I voluntarily assume all risks associated with participation in these activities.


    I am encouraged to consult with my physician before participating in any activities if I have concerns about my physical health or ability to participate.


    By signing this form, I voluntarily assume full responsibility for any injuries, loss, or -damage that may occur to myself during the Retreat, whether caused by my own actions, inactions, or the negligence of others.


    Please inform the retreat organizer if you have any medical conditions or dietary restrictions that need to be considered while planning the Retreat activities or meals.

    I understand that I am expected to drink responsibly during the retreat. Any consequences resulting from excessive alcohol consumption are solely my responsibility.


    If I choose to leave the retreat property alone after the retreat has started or arrive in advance and engage in solo activities, I do so at my own risk. The retreat organizer is not responsible for my safety or wellbeing during those periods.


    I acknowledge that missed flights or issues with third-party transportation (including taxis, ride-shares, or airline delays) are not the responsibility of the retreat organizer.


    Consent to Photographs and Media Use
    I understand that photographs, video, or audio recordings may be taken during the Retreat. I consent to the use of my image, voice, and likeness for promotional or educational purposes, which may include social media, websites, printed materials, and other marketing materials created by the Organizer.

    If I do not wish to be photographed or recorded, I agree to inform the retreat organizer at the time of arrival.


    Code of Conduct and Behavior Expectations
    To ensure a positive, safe, and respectful environment for all guests, I agree to follow the Code of Conduct during the Retreat, which includes but not limited to:

    Respect for Others: I will treat all fellow participants and staff with kindness, respect, and integrity, maintaining confidentiality in group discussions.
    Physical and Emotional Boundaries: I will respect the physical and emotional boundaries of others, and communicate any concerns or discomforts promptly to retreat leader (Mia Bolden).


    Active Participation: I agree to engage in the Activities to the best of my ability, but I understand I have the right to opt-out of any activity if I feel it is unsafe, uncomfortable, or outside of my physical or emotional capacity.


    Avoid Hearsay: Refrain from engaging in gossip or negative talk about fellow participants, facilitators, or retreat staff. If you have a concern or issue, address it directly and respectfully with retreat leader, Mia Bolden.


    Responsibility for Personal Belongings: While the retreat organizers will make every effort to ensure a secure environment, participants are responsible for their own personal belongings. The retreat is not liable for lost or stolen items.


    Embrace Diversity: The retreat welcomes individuals from all walks of life, including different cultures, races, sexual orientations, and spiritual beliefs. Engage with others in an open-minded, inclusive, and non-discriminatory way.


    Respecting the Retreat Space: Treat the retreat venue and all shared spaces with care. Keep your personal area tidy, and respect common areas. Dispose of trash properly and maintain a sense of collective responsibility for the environment.


    Tobacco Use: If smoking is permitted in certain areas, please do so in designated smoking zones, away from group activities, and be mindful of the comfort of others. Dispose of tobacco products and other waste responsibly.


    Listening with Presence: Practice active listening when others are speaking. This means being present, giving your full attention, and refraining from interrupting. Every person’s voice and experience is valuable.


    Respectful Speech: Speak with kindness and clarity. Avoid using language that could be hurtful, derogatory, or aggressive. If you have concerns or feedback, offer it in a constructive and respectful manner.


    Mobile Phones and Devices: To encourage full presence in the retreat experience, please keep mobile phones and other electronic devices on silent or turned off during group sessions, workshops, and activities. If you need to take a call or handle something urgent, step outside the group space to do so.


    Physical Health: Please inform the retreat organizer, Mia Bolden directly,  of any pre-existing medical conditions, physical restrictions, or special needs (e.g., mobility issues, allergies, or dietary preferences) ahead of time, so we can ensure your comfort and safety during the retreat.


    Mental and Emotional Health: This retreat may be an emotionally intense experience. If you experience emotional discomfort or find that you are struggling with any feelings during the retreat, please reach out to a facilitator, retreat leader, or on-site staff member for support.


    Emergencies: In the event of a medical or other emergency, inform the retreat leaders or staff immediately.


    Acknowledgment of Non-Professional Counseling
    I understand that the activities and workshops offered at the Retreat are for personal development, relaxation, and wellness, and that facilitators and retreat leaders are not licensed mental health professionals unless specifically stated. Any emotional support provided is intended for general well-being and personal growth. I acknowledge that I am responsible for seeking professional counseling or medical assistance if needed.


    Indemnification
    I agree to indemnify and hold harmless the Organizer, its staff, contractors, and facilitators from any claims, damages, or liabilities arising out of my participation in the Retreat, including but not limited to personal injury, medical emergencies, lost or damaged property, or emotional distress.


    Retreat Refund Policy
    Thank you for choosing to participate in Mia Bolden’s “Relax, Reset, Renew Women’s Empowerment Retreat”! We are committed to providing a transformative and enriching experience for all our guests. To avoid any misunderstandings, please carefully read and acknowledge our No Refund Policy.


    This No Refund Policy applies to the full retreat package, including all activities, excursions, meals, and any other services included in the retreat. Flight reservations and costs are not included in the retreat package and are the responsibility of the participant.

    a. No Refunds for Retreat Package
    Once your reservation is confirmed and payment is made, No Refunds will be issued for any reason. This includes, but is not limited to cancellations by the guest. If you cancel your attendance for any reason, we regret that we cannot provide a refund. 


    Early departure: If you choose to leave the retreat early, for any reason (personal, medical, or otherwise), No Refund will be issued for any portion of the retreat.
    Missed activities: If you miss any part of the retreat, such as workshops, group sessions, or other scheduled activities, there will be no refund for those missed portions.It is important to ensure that you can fully participate in the retreat before booking your spot.

    b. No Refunds for Activities, Excursions, and Meals
    The Relax, Reset, & Renew Women's Empowerment Retreat package includes a variety of activities, excursions, workshops, and meals. Once your reservation is confirmed, No Refunds will be issued for the following:
    Activities and Excursions: If you miss or choose not to participate in any of the scheduled activities or excursions, there will be No Refund for those experiences. This includes any optional excursions or experiences offered as part of the retreat package.
    Meals: Meals are included as part of your retreat package. If you miss a scheduled meal or choose not to attend a meal, there will be No Refund for any missed meals. If you have dietary restrictions or preferences, please notify the organizers in advance, but No Refunds will be given if you choose not to participate in meals or specific offerings.
    Program Modifications: If a particular activity or excursion is canceled, altered, or rescheduled by the organizers (due to weather conditions, unforeseen circumstances, etc.), No Refunds will be provided. We will do our best to offer alternative activities or experiences, but refunds will not be issued for any changes in the program.

    c. Flight Reservations and Travel Costs Exclusion
    Please note that flight reservations, transportation, and any travel-related costs (including flights, car rentals, travel insurance, or any other travel expenses) are not included in the Relax, Reset, & Renew Women’s Empowerment Retreat package and are not refundable by retreat organizer, Mia Bolden.

    Flights: Participants are responsible for booking and paying for their own flights to and from the retreat location. If a flight is canceled, delayed, or if you are unable to attend due to travel-related issues, no refund will be issued by the retreat organizers for any associated costs.
    Travel Insurance: We strongly recommend that you purchase travel insurance to protect yourself in the event of an emergency or unforeseen circumstance affecting your travel plans. The retreat organizers are not responsible for travel disruptions, flight cancellations, or any travel-related costs.

    *At the discretion of the retreat leader, if a refund is issued under extraordinary circumstances, please allow up to 90 days for processing.

    Right to Expel
    While we strive to create a welcoming and supportive environment, the retreat leaders reserve the right to remove any participant from the retreat if their behavior is disruptive, disrespectful, or poses a safety risk to themselves or others. This includes but is not limited to:

    *Harassment or inappropriate behavior toward other guests or staff.

    *Violation of retreat policies regarding code of conduct expectations and prohibited behaviors.
    *Continuous disruptive behavior that undermines the group process or retreat experience.

    Participants removed from the retreat for violating the Code of Conduct will not be eligible for any refund.

    Acknowledgment and Agreement
    By signing below, I confirm that I have read and understood this Waiver and Release Form in its entirety. I voluntarily agree to the terms outlined above and acknowledge that I am participating in Mia Bolden’s “Relax, Reset, & Renew Women’s Empowerment Retreat at my own risk.
