Kids Singing Group Application Form

We’ve seen so many talented kids with incredible voices but very few opportunities to shine. With all the doors God has been opening in my own journey, I want to use this moment to give back — by helping the next generation share their gift through music.

This form is for parents of children ages 7–12 who would like to be part of our Kids Collective under Flow Masters Records. Please fill out the form and upload a short video of your child introducing themselves and singing a short song. We can’t wait to discover and support the stars of tomorrow!

Submission Requirements:

1. Introduction Video - A short video of your child introducing themselves (e.g., name, age, favorite song, why they love singing

2. Singing Video - A 30-60 second video of your child singing any song of their choice (a cappella preferred)

3. Download, sign, and upload this consent form along with the video submission.

4. Submission Instructions: Upload the signed form and videos to the website (weareflowmasters.com) or email them to info@weareflowmasters.com with the subject line: 'Kids Singing Group - [Child Name]'.

Flow Masters Records will be selecting the best 3 children to be part of this special singing group. If your child is selected, we will contact you directly with further details about rehearsals and recordings. Due to the number of submissions, only selected participants will be notified.

Participation Rules & Expectations:

- Commitment to rehearsals and recordings is required.

- Respect and positive behavior towards peers and instructors are expected.

- Parents must provide transportation for rehearsals and recording sessions.

- Children must dress appropriately for performances and promotional materials.

- Any absence must be communicated in advance.

- All final creative decisions regarding music and production will be made by Flow Masters Records. Failure to comply with these rules may result in removal from the group.

Parent/Guardian Consent & Waiver

By signing below, I grant permission for my child to participate in the Kids Singing Group under Flow Masters Records.

I understand that:

-My child’s voice, image, and likeness may be recorded, photographed, and used for promotional purposes.

-The recordings may be distributed via digital streaming platforms, social media, and other promotional avenues.

-I waive any rights to financial compensation for the use of my child’s recordings or images.

-I release Flow Masters Records from any liability related to participation in this project.

-I affirm that I have read, understood, and agree to the participation rules.
