GROWTH 2025 Conference Scooter & Wheelchair Rental

Scooters and wheelchairs will be available for pick-up after 2:00 p.m. on July 23, 2025, and must be returned by 2:00 p.m. on July 31, 2025.

Please be aware that there are only a limited number of devices available. Reservations will occur on a first-come, first-served basis. Devices must be returned to the staging area by 2:00 p.m. on July 31, 2025. A late fee of $100 will be applied to devices returned after this deadline. Additionally, if you require a new key during your rental period, a $50 key replacement fee will be charged.

Liability Waiver for Scooter and/or Wheelchair Rentals
Agreement between the Pentecostal Churches of the Apostolic Faith International organization (Party B) and the Renter (Party A):

1. Purpose and Assumption of Risk
Party A acknowledges that they are voluntarily renting a scooter from Party B. Party A assumes all risks associated with scooter and/or wheelchair use, including but not limited to accidents, injuries, and property damage.

2. Release of Liability
Party A hereby releases Party B, its officers, employees, and volunteers from any liability arising from scooter and/or wheelchair rental, use, or maintenance. Party A agrees not to hold Party B responsible for any harm or injury incurred during scooter use.

3. Safety Guidelines: Party A agrees to follow all safety instructions provided by Party B.

By signing below, Party A acknowledges that they have read and understood this waiver and voluntarily accept its terms.
