Transcript Request Form

Starting January 1, 2025, a $10.00 processing fee will be charged for all transcript requests. Students requesting transcripts must complete the transcript request form and pay the $10.00 fee prior to transcripts being released. Transcripts will be processed within 72 hours of receipt of payment. Please complete the following information:
