Sacred Heart Church Parking Lot (Southwest corner, near the Victory Center)

10:30am Load the Bus at Sacred Heart Parking Lot

11:30am Arrive at Basilica - Holy Hill and get prepped for Mass (counts as your Sunday Obligation) and tour.

12:00pm - Mass

1:00pm - Lunch

1:30pm - Speaker

1:50pm - 3:30pm - Tour/Games

Around 4:30 - 5:00 pm - arrive to Sacred Heart Parking Lot (Southwest corner)

(1) We ask that you bring $10.00 cash (if able) for Lunch {if you would like to drop off before hand you can}

(2) Comfortable, weather-appropriate attire, but suitable for Mass.

FIELD TRIP CONSENT AND RELEASE: In consideration for my child/ward’s participation, I agree to reimburse and indemnify the parish/school for all reasonable legal and court fees incurred by parish/school in defending a lawsuit that I or my child/ward may bring against the parish/school Which relates to the above named activity if the parish/school is found not legally liable by the courts and prevails in the lawsuit. If the parish/school is found legally liable for injuries sustained by child/ward, this paragraph will not apply. I certify that I have an understanding of this agreement and any risks and hazards associated with the activity described above that my child/ward will be participating in. I further understand that I had the opportunity to fully discuss this agreement with a representative of the parish/school to clarify any concerns or questions about the activity or this agreement that I may have had.
Photo Permission

The Holy Family Parish staff member will be present to take photos or videos of the students for use in program displays, the parish bulletin or website, or other publicity-related publications. These materials will only be used for appropriate purposes and names will not be listed. It will be assumed that you give permission for your child to be photographed or videotaped unless you provide a statement via e-mail to Sabina Carter at angieb@hffdl.org. She will reply back to verify that your wishes are understood. If you do not receive a response back within 48 hours, she did not receive your e-mail so please try again. Thank you.
