Surgical Technology Program | Enrollment Agreement
The Surgical Technology Program at the Utah State University Blanding (USUB) and USU Eastern, is four semester (60 weeks) or 56 credits. To be accepted into/continue in the program, the student must have the following completed:
- Applied for admittance and been accepted into USU
- The required pre-requisites courses
- Submitted a completed application to the Surgical Technology Program
- A background check and drug screen (negative) through CastleBranch Records Management Company, (portal.castlebranch.com/ut61 The cost of performing this is approximately $91.00.
- BLS certification will be provided during the first semester of study
- Blanding students pay LaPriel Begay, USU Blanding Cashier, $45.00 for a drug screen and purchase only a background check through CastleBranch. Moab students work with USU Moab to obtain a drug screen through Elwood Staffing. They also only order background check through CastleBranch.
- Completed required vaccinations
- Completed required pre-requisite courses with an overall GPA of 3.0 or higher
Program tuition, including registration fees/books for Surgical Technology Program is _____ Scrubs and closed-toe shoes will be required and the cost is the responsibility of the student.

The Surgical Technology (ST) program runs consecutively every fall and spring semesters, with new cohorts beginning in the fall. Graduation is once a year in April. Once the student has successfully completed all course requirements, they will receive an Associate of Applied Science degree in Surgical Technology, and will be eligible to take the national certifying examination through NBSTSA. In addition, the student MUST apply for and pay for graduation through Utah State University by the stated deadline.

The student has the option to attend this course in a blended format. The following courses are available for delivery via IVC to outreach learning centers:

- SURG 1050
- SURG 1060
The USU ST Program does not guarantee job placement or employment upon completion of the program.
