Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by The Art of Music Studio during the selected camp. In exchange for the acceptance of said child’s candidacy by The Art of Music Studio . I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Harvest Family Church, The Art of Music Studio. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Harvest Family Church, The Art of Music Studio. including all teachers and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all activities.
Student Registration Agreement for Summer 2026

I understand and agree that my student will pay a $25 non-refundable registration fee per camp that will hold their spot in the camp.

I understand and agree that the Summer Camp Registration fee must be paid when the student register, and full payment for the camp must be paid in full 14 days before the first day of the camp (Monthly or weekly payment can be set up when you register).

I understand and agree that if I drop a Camp for any reason, it must be done 30 days before the 1st day of the Camp to receive a full refund for the Camp, minus the $25 registration fee. If the Camp is dropped 14-30 day before the Camp, I will receive 50% of the tuition paid. If a Camp is dropped 14 days or less before the Camp, there will be no refund for the Camp since supplies and teachers have been fully prepared at that time. I understand and agree that if I need to drop a Camp, I will contact The Art of Music Studio office by phone or text and also send a notice by email to registrations@theartofmusicstudio.com.

I understand and agree that The Art of Music Studio only accepts auto pay for tuition, by automatic bank account payment, debit card payment, credit card payment, or a set of post dated checks as payment. Full payment for a Camp can be paid by check, credit/debit, or cash. I understand and agree that there will be a $3 per $100 convenience fee added to all Credit card or Debit card tuition payments (i.e. for $95 group class, there would be a $2 convenience fee added to the $95 tuition).

I understand and agree that no make-up lessons will be given if students miss any days of scheduled Camps. (We will work with you to set up Private Lesson Camps and Buddy Camps around your vacation times if you let us know when you register). I understand and agree that no refunds are given for missed lessons.

I understand and agree that I as the Student, or I as the Parent or Legal Guardian of the Student, (if the student is a minor) waive the right to any legal action for any injury sustained on Harvest Family Church property resulting from normal lesson activity or any other activity conducted by the students before, during or after lesson time.

I understand and agree that The Art of Music Studio is not responsible for providing before or after class care for students. I understand and agree to remain in the building during the lessons/classes if my student is under the age of 5yr. I understand and agree that I will not leave my student at the school for excessive time before or after lessons.

I understand and agree that any and all songs (words, melodies, chords, or any other parts of songs) or other written material for classes and during camps belong to The Art of Music Studio.

I understand and agree that my student is not permitted to roam the building, bathrooms, play on the stairs, etc. at any time, and must stay seated in the waiting room before and after lessons.

I understand and agree that The Art of Music Studio is granted permission to take photographs of students to use in brochures, websites, posters, advertisements, videos, or any other promotional materials the school creates and to copyright such photographs in its name.
