1. Assumption of Risk
I acknowledge that participation in inline/roller hockey—whether pickup games, informal skirmishes, practices, tournaments, travel, or any other FRHF event—carries inherent risks, including but not limited to: sprains, fractures, head injuries, permanent disability, or death. These risks may arise from my actions or the actions of others, surface conditions, equipment failure, or environmental factors.
I understand these risks and voluntarily choose to participate, fully accepting all consequences. I certify that I am physically fit and capable of engaging in strenuous physical activity.
2. Personal Responsibility & Inspection
I agree to:
-Inspect playing surfaces and equipment prior to participation
-Refrain from play if I believe conditions are unsafe
-Act in a manner that prioritizes the safety of myself and others
-Accept full responsibility for managing my physical condition and any necessary medical care
3. Personal Insurance Acknowledgment
I affirm that:
-I have obtained and will maintain my own personal sports or accident insurance through a provider such as AAU Hockey, USA Hockey, or an equivalent
-I understand and accept that FRHF does not provide insurance coverage and does not verify the existence or validity of individual player coverage
-By signing this waiver, I confirm I have secured such coverage, and I release FRHF from any responsibility related to my failure to do so -I understand and agree that FRHF has no responsibility to verify, monitor, or enforce the existence or validity of my personal insurance coverage. I alone am responsible for securing and maintaining this coverage.
4. Waiver and Release of Liability
In consideration for being permitted to participate in any FRHF activity, I, for myself and on behalf of my heirs, executors, representatives, successors, and assigns, hereby fully and forever release, waive, and hold harmless the City of Fresno and the Fresno Roller Hockey Foundation (FRHF), its officers, directors, agents, volunteers, participants, and affiliates from any and all claims of liability, including but not limited to:
-Personal injury
-Property damage
-Illness
-Death
-Financial loss arising from or related to participation in any FRHF event, whether on or off the rink, and whether caused by negligence or otherwise.
5. Indemnification
I agree to indemnify and defend FRHF against any claims, actions, damages, or liabilities (including legal fees) resulting from my participation or conduct in any FRHF-sanctioned activity or event.
6. Media Release & Usage Restriction
I grant permission for FRHF to use photographs or video of me taken during FRHF events for documentation, promotional use, and social media.
I understand that any images or footage captured at FRHF events may not be used to promote personal brands, businesses, or unaffiliated teams or organizations without express written consent from FRHF leadership.
7. Tournament Participation & Commitment
I understand and agree that I may be invited to participate in local or out-of-town tournaments organized or supported by FRHF. My participation is subject to the following:
-Selection is not guaranteed. Final decisions are made solely by FRHF leadership
-Tournament fees must be paid in full by the deadline provided, unless otherwise negotiated and agreed upon
-All payments are non-refundable unless FRHF leadership approves an exception
-FRHF may substitute players for any reason including injury, absence, or conduct
-I release FRHF from liability related to tournament travel, lodging, or venue risks
8. Severability
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full effect.
9. Code of Conduct & Removal Policy
All FRHF participants are expected to conduct themselves in a manner that reflects the values of discipline, respect, and community. The following behaviors will not be tolerated:
-Verbal abuse, threats, or hostile behavior
-Excessive arguing, fighting, or unsportsmanlike conduct
-Repeated disruption of games or refusal to follow rules
-Harassment, discrimination, or misconduct
-FRHF leadership reserves the right to immediately suspend, remove, or permanently ban any participant for conduct deemed detrimental to the safety, integrity, or energy of FRHF events. All such decisions are final, and no refunds will be issued in the event of removal.
10. Signature & Consent
By signing this waiver, I acknowledge that I have read and fully understood all ten sections. I agree to comply with all FRHF rules, policies, and conduct expectations, and I voluntarily accept all associated risks and responsibilities as outlined in this agreement.
