I give permission for my child(ren) listed above to participate int FBYouth Winter Jam Activity this includes travel to and from event. Although I understand that First Baptist and Sponsors will make reasonable effort to provide a safe trip, I am fully aware of the special dangers and risks inherited in traveling and participating in this activity. I understand that during this experience my child(ren) will be under supervision by First Baptist Leaders/Sponsors. I am hereby and voluntarily release, discharge, waive and relinquish any and all loss or damages or actions or causes of action for personal injury, property damage occurring to my child(ren) during his or her involvement with First Baptist Church Activities. In the event of an accident or illness, I understand that reasonable effort will be made to contact the parent immediately . However, if I am not available, I authorize First Baptist - Fremont to secure emergency medial care as needed.
