Your Name is a Gift - Personalization Info Form

Use this form to give information needed to Customize the Story Book
This 20 page full color, custom childrens story book (11" x 8.5") explores how your family uniquely named the child, prays for them, and helps the child discover the saints, people in the bible or bible elements that inspired their name.

Included in the cost of the book is to upload one color high resolution photo to include in the book, and made into an illustration.

Additional cost: Upload one high resolution image for custom front cover.

Fill in the information below to customize the book specifically for your child. We may be in touch with you if we have any questions.

Complete and submit one form per book ordered.

ORDER TIME LINE:

1) Complete Customization with the Personalization Info Form & Submit.

2) Complete Order with payment.

3) Within 2 business days, receive proof of customized pages to approve. Make sure the email from Sharron@SBLearningDesigns.com does not go to your spam or junk mail folders.

4) Upon approval of proof, printing time is approximately 10 business days. Expedited printing pricing is available. Expedited printing time is 5 business days.

5) Shipping will come directly from the printing company, standard shipping delivery through USPS of 5-10 business days, at no extra charge (included with cost of the book).

Expedited shipping fees are available and can be added after the purchase of the book.

Approximate "standard" total time from order to delivery is approximately 3 - 4 weeks, from approval of the proof.
