  • Review the Policy:

    The Provost coordinates the collection, dissemination, and recording of student grievances and complaints that are academic AND non-academic. Prior to completing this form, please review the Student Handbook and adhere to the guidelines outlined in this policy.
  • Submitting your complaint:

    Students are encouraged to resolve complaints at the appropriate level of dispute, however, should this approach fail or be inappropriate, you may use this form to submit a written complaint which will be kept in strict confidence. All pertinent information should be included, such as parties involved, witnesses, any attempts to resolve the complaint, along with your desired resolution. Anonymous submissions are also accepted.
  • Resolution of your complaint:

    Complaints are respected and honored; no reprisals or retaliation shall be invoked against any student for processing, in good faith, an informal or formal complaint or for participating in any way in these complaint procedures. Students who choose to appeal a complaint are granted that right and may pursue further resolution through the Seminary.
