Authorization for Medical Treatment: I hereby give permission to notify emergency medical services in the event of an emergency. I understand that I will be contacted immediately following the notification of emergency services. Furthermore, I understand that any medical cost incurred will be my responsibility.

Authorization to attend the selected camps:

I give our permission for the student listed above to participate in Seton-organized athletic camps, realizing that such activity involves the potential injury, which is inherent in all sports. I acknowledge that even with the best coaching, use of the most advanced protective equipment and strict observance of rules, injuries are still a possibility. On rare occasions, the injuries can be so severe as to result in total disability, paralysis, quadriplegia or even death.

I acknowledge that I have read and understand this warning.

Authorization for photographs and videotaping:

I hereby give my permission for Seton Catholic Prep High School to photograph and/or videotape my child for promotional purposes.

Waiver:

I waive and release any and all rights and claims I have against Seton Catholic Prep or its representatives for damages that may be sustained by my child. I acknowledge that the above activity may pose some risk of personal injury or death and that I undertake and assume this risk for my child. On behalf of myself and my child, I further waive and release the Seton Catholic Prep, the school facility, any insuring entity of the above, and their directors, board members, officers, employees, volunteers, agents, representatives, or assigns, as well as the activity sponsors, from any and all liability, including, but not limited to, liability arising from negligence or fault of the entities or persons for any injury or disability which may occur as a result of my child’s participation in the above activity. I am assuming all risks on behalf of myself and my child that may arise from negligence or carelessness on the part of any of the persons or entities being released, as well as from defective equipment, real property or personal property that is owned, maintained or controlled by the above persons
