SAFETY NOTIFICATION

Student Day at Live Trench Training is an active job site. Heavy equipment will be in use. Protective Safety Equipment will be provided (hard hats, vests, eye protection). Students must wear closed toe shoes and denim jeans (no sandles, crocs, or shorts). Students not dressed accordingly will not be permited on the jobsite.

Students will be required to provide a signed waiver to attend. The waiver will be emailed to the school primary contact indicated in this form for distribution to participating students. If the student is a minor, a parent/guardian must sign the form.
