• 8:30AM:   Rules Meeting (Judges)
  
• 9:00AM:   All divisions start the weapons competition
• 10:30AM:  All divisions start the forms competition
• 1:00PM:  All divisions start the sparring competition
• 4:00PM:  Tournament finished -- time to go home

Please arrive at 8:30AM for line-up and rules

Competitor Fees:

• Before Wednesday May 27th (Pre-Registration) :
  • 1-3 categories --  $80.00
  • Additional family member competing:  $70
  • Spectator tickets -- no charge
• After May 27th:
  • 1-3 categories --  $95.00
• At the door:
  • 1-3 categories -- $95.00

Amateur Athletic Release of Liability

 

In consideration of being allowed to participate in any way in the 48th Annual CT State Tang Soo Do Championships, the undersigned, or parent or guardian of a minor child participant, agrees to inspect prior to participating the facilities and equipment used and if the participant believes anything is unsafe he or she should immediately withdraw from the competition and refuse to participate.  Further acknowledges and fully understands that each participant will be engaging in activities that involve risk of serious bodily injury, including permanent disability and death, and severe social and economic losses which might result not only from their actions, inaction or negligence of others, the rules of play, or the condition of the premises or any equipment used.  Further, that there may be other risks not known to us or not reasonably foreseeable at the time, agrees to assume all the forgoing risk and accept personal responsibility for the damages following such injury, permanent disability or death.  Releases, waives, discharges and covenants not to sue the West Haven Academy of Karate, Inc., its affiliated clubs, the Jeffery P Hazell Athletic Center, the officials, instructors, sponsors or other participants, all of which are hereafter referred to as "releasees", from any and all claims, demands, losses or damages on account of injury, including death or damage to property caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise.

WE HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.

If the competitor is a minor, the signor must be a parent or legal guardian.

Register each student separately (even if in the same family).
