Membership Registration Form

This membership form is primarily for Lifeguards, Associates and Junior Surf.

Personal information collected is for use by both SLSNZ and the Club, including in assessing suitability for membership; health and safety record, disciplinary actions, inter-club activities, SLSNZ events and transfer of membership. Reasonable security safeguards will protect member’s stored data which will only be shared at a Board or administrative level if requested by SLSNZ or other Club’s Board Members.

Overview of Membership Types:
Age is determined as at the 1st October of the current year

• Lifeguard Membership (14+ years) - $60
Lifeguard Membership is a full Club membership that includes access to Club facilities, training and development courses, Fit for Purpose and sports coaching & competitions.

• Active Lifeguard Membership with subsidy (14+ years) - $30
Lifeguards who have completed 30 hours of volunteer patrolling/active service during the previous season are entitled to a $30 reduction in registration fees. You can check your hours on your SLSNZ login if required.

• Rookie Lifeguard Membership (12-13 years) - $60
Rookie Lifeguard membership can be included in the Junior Surf membership or stand-alone. Membership includes access to training and development courses and sport.

• Lifeguard Candidate Training and Membership (14+ years) - $170
Includes: Training days, workbooks, pool entry, lifeguard exam and current season membership fee. Once candidate has secured the Lifeguard Award the fee also includes a lifeguard uniform - polo shirt and shorts.

• Junior Surf Membership (5+ years) = 1 Child & 1 Parent/Associate membership - $110
A minimum of one adult must join the Club as an Associate Member (or Lifeguard Member) with Junior Surf membership.

• Additional Junior Surf Athletes (5+ years) - $65
To add any additional children joining Junior Surf to your membership only. If you have more than one child taking part in Junior Surf, you only need one Associate Member per family.

• Associate Membership - $45
Non-competing members, parents/guardians or volunteers. Associate members can access club facilities and vote at the Annual General Meeting.

• Social Membership - $25
Public membership to support the Club and to enable access to enjoy the Club's bar and kitchen known as 'The Deck'.

JUNIOR SURF - Additional Information (a must read for new members!)
Junior Surf can seem daunting to new members who have not been a member of a Surf Club before and who aren't sure of the logistics. There is a lot of information available to you which will explain how it all works so please take some time to review this.

Junior Surf Club Membership
This form you are completing is for Waihi Beach Club Membership. Both the child/ren enrolling and one associate adult needs to become a member. If you have one child, the number of members you are signing up for would be two. Please enter all the details for the Junior Surf athlete AND the parent member.

Example for Membership for Junior Surf:
Member 1 (Parent) = select: Junior Surf Associate Parent $0 (*inc in Junior Surf Athlete)
Member 2 (First Child) = select: Junior Surf Athlete $110 (*inc Associate Member)
Member 3 (Second Child) = select: Additional Junior Surf Athlete $65

*Lifeguard & Junior Surf Club Membership - If you are a Lifeguard Member with a Junior Surf athlete, please select the Lifeguard Membership option for yourself and then the Additional Junior Surf Athlete option for your child so you are not also charged for an Associate Membership.

Surf Life Saving New Zealand Membership:
Please note if you are new to Surf Lifesaving you will also need to register with Surf Lifesaving New Zealand for an SLSNZ number. This form needs to be completed for all children starting with Junior Surf and for their associate parent. It is a very short form with no added costs and only needs to be done once to give you a member number for life. This number is needed for insurance purposes, competitions and to be included in the SLSNZ database.

Accommodation Agreement (for lifeguards only)
X 14 to 15 year olds are not permitted to stay in the accommodation block un-supervised and are not required to complete the accommodation agreement at this time

X 16 to 17 year olds who wish to stay must firstly seek approval from the club, and their parent or guardian must then provide written proof of permission

ü18+ year olds by signing the accommodation agreement on this form, you acknowledge and agree to the conditions of residence whilst staying at Waihi Beach Lifeguard Services facilities.

Terms and conditions of staying in the accommodation block:

● All Accommodation requests must be emailed to the office, with at least 24 hours notice.

● All lifeguards MUST sign in on the designated whiteboard at the bottom of the stairs on arrival and when exiting the building. This is a FIRE REGULATION.

● Rooms must be separate for males and females.

● No visitors or non-members are allowed in the accommodation area.

● No noise after 10 pm. Close doors quietly, no talking or running in hallway as guards patrol in the morning.

● Leave the bedroom in a clean and tidy manner, remove rubbish and no wet gear in rooms.

● Be responsible for cleaning the bathroom, kitchen and hallway with other lifeguards staying in the block.

● No contraband is to be brought into the Club.

Responsibilities:

● Ensure the building, windows, and doors are secure at all times.

● Treat the accommodation block, gear and equipment with respect.

● Report any damage via the QR code located through-out the building or to your Patrol Captain, Board Member or Administration Manager.

● Pay for any damage to property caused by the lifeguard.

● Pay for any costs associated with a lack of cleanliness left by the lifeguard.
