** DEADLINE FOR ENTRY IS MAY 10 **

Entry forms must be received, with payment, by midnight May 10. The non-refundable entry fee for provincial entries is $50 per provincial class and $100 per Canada West class. Please supply complete and accurate information.

Before beginning to fill out this form, please have all of the following information and be prepared to make payment, as you won't be able to save an incomplete form and return later to complete it.
- The Provincial class number and name in which you advanced from your district festival.
- Complete repertoire information.
- Accompanist name (if applicable).
Payments are made through PayPal, with the option to use a credit or debit card.
