In consideration of the youth named herein and the opportunity for that youth to voluntarily participate in the Jaguars Youth Girls Basketball camp, I hereby release and discharge Hilliard Bradley Girls Basketball staff, Hilliard City Schools, and the instructors of any injuries or illnesses which may result because of participation in this camp. By completing this form, you, on behalf of yourself and your daughter or any other persons for whom you are legal guardian, confirm: (1) That you understand the statements contained on this form; and (2) That you release camp coaches and the Hilliard City Schools from any claims, liability, injury, or damages occurring during this camp.
Confirmation BY ACKNOWLEDGING AND SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
