    Registration is REQUIRED to participate in any of the following activities: Marching Band/Color Guard Mini-Camp (May 2025), Orchestra/Choir/Band Camps (fall 2025), Marching Band/Color Guard, Orchestra, Choir, Jazz Band, Non-Marching Band.
  • This form will take less than 5 minutes to complete. 

    At the end of this form, you will be asked for a voluntary contribution. These contributions are crucial to keeping programs available for our students.

    Please note that low contribution rates in recent years have necessitated program cuts and put the sustainability of these programs at risk.


    Here are the requested contribution amounts:

    • Band (non-marching only): $375
    • Choir: $250
    • Jazz Band: $525
    • Marching Band/Color Guard: $1300
    • Orchestra: $450
    • Winter Programs (Guard & Percussion): $1350

  • If a serious emergency arises, it might be necessary for a physician to attend to your student before someone can contact you. Such care can be provided only if you sign the AUTHORIZATION TO TREAT below. In the event of illness or injury, I do hereby consent to whatever X-ray, examination, anesthetic, medical, surgical, or dental diagnosis or treatment and hospital care are considered necessary in the best judgment of the attending physician, surgeon or dentist and performed by or under supervision of a member of the medical staff of the hospital or facility furnishing medical or dental services.

    Further, as a parent or legal guardian of a student attending these school-sponsored trips, I understand that it is not the same physical environment as a traditional school. There may be certain inherent hazards including natural and manufactured conditions at the site which may result in physical injury, harm, damage, or death. 

    I understand that there is no warranty or guarantee of my student's safety or security. I hold the LGSUHSD, Los Gatos Music Boosters, and the officers, agents, and employees of the above harmless from any and all liability or claim which may arise out of or in connection with my student's participation in the school-sponsored trip or activity and I waive ALL claims against the school district for injury, accident, illness or death occurring during or by reason of my student's participation (ED CODE 35330). 

    First aid: In the event that your student requires minor first aid, a school-sponsored coach, teacher, chaperone, or administrator will render treatment as necessary in accordance with standard first-aid practices. 

    Transportation: I may be contacted to transport my student home due to illness or misconduct.

    Media Approval: By choosing to participate in the LGHS music program, students and parents should understand that photos, videos, and live broadcasts (collectively "the Recordings") may be taken and used to publicly promote the program. This may include using the Recordings in publications, posting on the LGHS and/or Music Boosters websites, and/or posting on social media. Individual students will not be named unless permission is obtained by the Music Boosters or Music Department.

  • VOLUNTEER!

  • The Los Gatos Music Boosters is led by a small group of parent volunteers who are passionate about providing all students with the best opportunities that music education has to offer. Because our program is dependent on parent volunteers, we need your help!

    All festivals, concerts, competitions, trips, and performances are produced in great part by volunteers, so we absolutely can not do it without your help. If you want your students to continue to thrive in their music education journey and create skills and memories to last a lifetime, we need each and every family to help however they are able. Every hour that you are able to contribute helps tremendously!

    There are lots of different options for varying levels of interest and ability and we encourage you to choose what works best for your family to contribute wherever they can. Please select the area in which you are most interested to serve. 

  • Most positions require volunteer screening. Please visit the district page for requirements which includes necessary forms, TB test information, and fingerprinting/background check instructions.

  • All students are required to be driven to competitions by parents or authorized adults. No students may drive themselves. Additionally, the carpool/driver form must be on file.

  • VOLUNTARY CONTRIBUTION

  • Donations have fallen short in recent years and the programs, festivals, & trips our students enjoy are at risk. 

    We will not be able to provide the same enriching experiences going forward if we do not have the necessary funding. In order to maintain the high quality music education our students enjoy, we need full contributions from as many families as possible.  We understand some are not in the position to contribute the recommended amount. Please remember any amount is appreciated. 

    Contributions support instructors, music, drill design, costumes, equipment, instruments and instrument repair, competition meals and fees, gas for our trailers, snacks, specialized staff, travel, and more!

  • DISCLAIMER: Nothing in the law prohibits the solicitation of voluntary contributions, donations, and fundraising by students and their families to support District and school site program, so long as monetary contributions and fundraising activities are NOT made mandatory for students and their families and there are no conditions placed upon the donation. (LGSHUSD BP3290). Because no goods or services are received in exchange for contributions, the full contribution is tax-deductible, but not refundable.

    We need as many families as possible to contribute. If your circumstances change, please consider donating later this year.  Every contribution is important, no matter the amount.
