Contracting Entities (CEs) should complete this survey after contacting one or more WIC clinics and confirming that their school/organization can support the WIC clinic(s) as summer meal sites in the 2025 WIC Pilot.

Survey answers will be used to identify the WIC clinics that have been matched with a CE and identify the clinics that still need a CE. Survey results with be shared with the Texas Department of Agriculture and Texas WIC.
