SCS & OCS Girls Basketball Camp Registration Form

June 2-6; for students entering grades 3-8
This summer the SCS Girls Basketball coaches & high school students will be hosting a basketball camp June 2-6 in the SCS gym. All students entering grades 3rd-8th for the 25/26 school year are invited to participate. Grades 3-5 will be from 9-10:30am, and grades 6-8 will run from 10:45am-12:15pm.

Registration is $50 per student and each student will receive a free t-shirt with their registration, if registered by May 14th.

I hereby give my approval for my child’s participation in any and all activities prepared by the SCS Cross Country Running Camp during the selected camp. In exchange for the acceptance of said child’s candidacy by the Hawaii's Pearls Soccer Camp, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless the SCS Basketball Camp and all its respective officers, coaches, agents, participants, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against the Sheboygan Christian School, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
