To offset some of the tryout expenses for facility rental and equipment, there will be a small fee for those trying out - $15 per athlete with a $50 family cap. Each athlete trying out will be given a tryout t-shirt as part of their registration fee. The shirts (pictured below) will have the Patriots' volleyball logo on the front and a square on the back where we can add a name or number to easily identify your athlete during evaluation. Payments may be made via Venmo, check, or cash.

If you did not homeschool your athlete this year but will for the 2025-2026 school year, please upload a letter stating such. Liability Waiver & Medical Release Statement

I understand and fully accept that sports involve risks and that accidents and injuries are common and ordinary occurrences. I understand there are additional risks for gathering in groups during a pandemic. I hereby agree to accept any and all risks of injury, illness, or death for me or my child and verify this statement by placing my INITIALS BELOW.

I hereby release and hold harmless the CVHAA Patriots Athletic Association, their volunteers, designated coaches, and program officials and supervisors from all liability, and from all actions or claims that my child or I now or hereafter have for damage or injury to my child, or to any person or property, resulting from the negligence or other acts of any employees or volunteers in connection with my child’s participation. I further agree that this waiver, release, and assumption of risks are to be binding on the heirs and assigns of the undersigned. I further agree to indemnify and to hold the CVHAA Patriots Athletic Association (its officers, employees, agents, and volunteers) free and harmless from any loss, liability, damage, cost, or expense which they may incur as a result of any injury and/or property damage that my child or I may cause or sustain while participating in this activity. The CVHAA Patriots Athletic Association does not disclose your nonpublic personal medical and financial information, except as required or permitted by law. The CVHAA Patriots Athletic Association also does not provide any medical or other insurance protection. I also hereby grant permission to the CVHAA Patriots Athletic Association to use my own or my child's pictures on their website, social media, or other promotional materials.
