Questions to Consider Before your Appointment
Are you constantly preoccupied with your weight, body shape, or muscles to the point where you can't stop thinking about it (or interferes with daily life)?
Have you changed your eating habits in order to change your weight, shape, or muscles?
Compared to other things in your life, how important is your weight, shape, size, or muscularity?
Sharing your Concerns
The concerns I would like to share with you around food and my body image are:
(eg. restricting food, avoiding certain foods or dietary changes, eating until I feel ill, feeling unable to stop eating once I start, body dissatisfaction, focus on muscle mass, etc.)
The physical concerns I would like to share with you include:
(e.g. recent weight loss or gain, trouble concentrating, dental issues, sleeping way less or way more, low energy, dizziness, etc.)
The emotional or mental health concerns I would like to share with you include:
