Outstanding Contribution to Health Education Award Nomination

The purpose of this award is to recognize exemplary efforts in the area of health education at the local or state level. Nominees for this award can be individuals or organizations. Nominees do not have to be members of SCAAHE or even reside in South Carolina, but should have advanced health education in South Carolina. Nominees may have received this award previously, but not for the same contribution being considered. There can be several Outstanding Contributions to Health Education Awards each year. Members of the SCAAHE Awards Committee are not eligible.

Nominations are to include written evidence below demonstrating how the nominee meets the criteria for the award.

Complete information demonstrating how the applicant meets the criteria for the award must be provided to the SCAAHE Awards Committee with the submission of the nomination form. Incomplete applications will not be considered. Submission deadline is August 30th.
