Cancellation & Rescheduling Policy – Simply Implants Institute
All course registrations are final. There are absolutely no refunds.
If you are unable to attend a scheduled course, you may request to reschedule your attendance to a future course by providing written notice at least 30–40 calendar days in advance. Rescheduling is subject to availability and must be confirmed in writing by the Simply Implants Institute team.
Failure to provide timely notice or receive confirmation will result in forfeiture of your course fee.
Institute Rights
Simply Implants Institute reserves the right, at its sole discretion, to deny registration, cancel or reschedule any course, or modify course content, instructors, or venues without prior notice. In the event Simply Implants Institute cancels or reschedules a course, registered participants will be notified promptly and given the option to transfer to another course date. No refunds will be issued.
Travel Expenses Disclaimer
Simply Implants Institute is not responsible for any travel-related expenses incurred by participants, including but not limited to airfare, hotel accommodations, meals, transportation, or incidental costs. This applies regardless of whether such expenses are prepaid, non-refundable, or subject to cancellation fees.
Participants are solely responsible for making their own travel arrangements and are strongly encouraged to purchase travel insurance to protect against any unforeseen changes, cancellations, or disruptions.
Media Release & Consent
By attending a course at Simply Implants Institute, participants acknowledge and agree that photos and videos will be captured by our professional videographer and photographer throughout the training.
By registering, you grant Simply Implants Institute the unrestricted right to use any images, video footage, or testimonials captured during the course for marketing, promotional, educational, and commercial purposes, including but not limited to use on our website, social media platforms, email campaigns, printed materials, and other media.
If you have specific concerns about participation in video or photography, please inform our team in writing prior to the course start date.
MALPRACTICE INSURANCE
1. I hold an active license to practice dentistry in the USA.
2. I agree to pay in advance for all continuing education programs and hands-on demonstrations.
3. I understand that when I give hands-on demonstrations, that my treatment will be free of charge to the person(s) on whom made, and I will receive no payment or other compensation for doing so.
4. Insurance Name.
5. Limits per claim, e.g. $1M
6. Have you informed your carrier in writing that you will be performing dental services in the State of Arizona?
7. Have you received written confirmation that your carrier will cover any alleged malpractice for services provided in the State of Arizona?
TEMPORARY DENTAL LICENSE ACKNOWLEDGEMENT (Required for out of state participants)
-I understand that if I am licensed in a state other than Arizona, I am required to obtain a temporary dental licence through the Arizona State Dental Board of Dental Examiners in order to participate in clinical activities during the course.
-I acknowledge that:
-Simply impalnts Institute does not process, facilitate, or oversee this application.
- It is solely my responsibility to apply for, complete, and secure the Arizona temporary dental license prior to the start of the course.
-I understand that this process of obtaining the Arizona temporary process can take up to 8 weeks, possibly longer.
- Proof of this licence must be submitted to Simply Implants Institute before I will be permitted
to participate in clinical sessions.
