I acknowledge that Metro Church and its affiliates are not responsible for any pre-existing medical conditions that may be affected or aggravated by travel.

I, the undersigned participant (and parent/guardian, if applicable), acknowledge and understand that traveling as a passenger in any vehicle—including a personal vehicle, a bus provided by Metro Church, or an independently driven vehicle—carries inherent risks, including but not limited to:

• Personal injury, death, or property damage.
• Accidents caused by the actions or ordinary negligence of the driver or other motorists.
• Unforeseeable third-party actions, such as reckless or impaired driving by others.
• Road conditions, weather, or environmental hazards.
• Mechanical failures or vehicle malfunctions.

I further acknowledge that Metro Church, Crosswalk Fellowship, their staff, leaders, volunteers, and affiliates are not responsible for the safety, maintenance, or operation of any personal vehicles, independently driven vehicles, or buses used for transportation. They do not verify, inspect, or maintain these vehicles.
By signing below, I voluntarily assume all risks associated with traveling in any vehicle, including those arising from the negligence of the Released Parties, except those resulting from gross negligence or reckless misconduct. I understand that my participation in this Event and use of transportation is entirely voluntary.

I further acknowledge that I have been advised to verify that the driver possesses a valid driver's license and insurance.

I acknowledge that participating in the Event involves inherent risks, including but not limited to physical injury, illness (such as communicable diseases like influenza or COVID-19), emotional distress, and, in extreme cases, permanent disability or death. These risks may result from my own actions or inactions, those of other participants, or the conditions in which the Event takes place. I fully understand and accept these risks, both known and unknown, and voluntarily assume them.

In consideration for being allowed to participate in the Event, I, for myself, and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Metro Church, its elders, pastors, directors, officers, officials, agents, volunteers, employees, and other participants, as well as any sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the Event (collectively referred to as “Releasees”), from any and all claims, demands, losses, damages, and liabilities, including any injury, disability, death, or damage to property, arising out of or in connection with my participation in the Event, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

However, I understand and agree that Metro Church is not responsible or liable for any medical expenses incurred as a result of my participation in the Event.

If I choose to drive independently or ride in a vehicle not provided by Metro Church or Crosswalk Fellowship, I assume all associated risks. I acknowledge that Metro Church, Crosswalk Fellowship, their staff, leaders, volunteers, and affiliates are not responsible for:

• The condition, safety, or maintenance of the independently driven vehicle.
• The driver’s actions, qualifications, or adherence to traffic laws.
• Any accidents, injuries, damages, or losses that occur while traveling in an independently driven vehicle.

I further understand that Metro Church and Crosswalk Fellowship have not encouraged or directed me to travel separately and that this decision is entirely my own.

I hereby grant Metro Church permission to use my likeness in photographs, videos, or other digital media ("photos") for any and all publications, including web-based materials, without payment or other compensation.

I understand and agree that all photos will become the property of Metro Church and will not be returned. I irrevocably authorize Metro Church to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. I also waive the right to inspect or approve the final product in which my likeness appears, as well as any right to royalties or compensation related to the use of these materials.

Furthermore, I release and forever discharge Metro Church from any claims, demands, or causes of action that I, my representatives, executors, administrators, or any other persons acting on my behalf may have in connection with this authorization.
