WAIVER FORM (PM ACADEMY)
Please read this document carefully. By participating in any activities organized or conducted by PM Academy, you acknowledge and agree to the following terms and conditions:
1. Assumption of Risk:
I, the undersigned participant (or parent or legal guardian of the participant), understand and acknowledge that soccer training involves certain inherent risks and dangers that cannot be eliminated, despite the best efforts of PM Academy. These risks include, but are not limited to, accidents, injuries, and illnesses that may arise from participation in soccer-related activities, such as running, jumping, kicking, and physical contact with other participants, equipment, or facilities. I understand that these risks may result in personal injury, property damage, or even death.
2. Release of Liability:
In consideration of being allowed to participate in the activities organized by PM Academy, I hereby release, waive, discharge, and covenant not to sue PM Academy, its employees, volunteers, agents, representatives, and affiliates (collectively referred to as "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including but not limited to bodily injury, personal injury, or property damage, that may be sustained by me (or my child) during or as a result of participation in the activities, whether caused by the negligence of the Released Parties or otherwise.
3. Indemnification:
I agree to indemnify, defend, and hold harmless PM Academy and its Released Parties from any and all claims, damages, losses, liabilities, costs, or expenses (including reasonable attorneys' fees) arising out of or related to my (or my child's) participation in the activities organized by PM Academy.
4. Medical Treatment:
In the event of an injury or illness during participation in PM Academy's activities, I authorize PM Academy and its representatives to obtain and provide necessary medical treatment, including but not limited to, first aid, emergency medical care, or hospitalization, as deemed necessary. I understand that PM Academy will make reasonable efforts to contact emergency contacts listed on the registration form in case of a medical emergency.
5. Use of Likeness:
I grant PM Academy and its representatives the irrevocable right and permission to use photographs, video recordings, and any other media of me (or my child) taken during the activities for promotional purposes, including but not limited to website content, social media, marketing materials, and publications, without any compensation or further consent.
6. Severability:
If any provision or part of this waiver and release of liability is found to be invalid, illegal, or unenforceable, the remaining provisions shall not be affected and shall remain in full force and effect.
7. I have carefully read this waiver and release of liability and fully understand its contents. I acknowledge that I am (or my child is) voluntarily participating in the activities organized by PM Academy and agree to be bound by its terms.
