Please Note:
Completion of this application does not guarantee adoption of a pet.
Applicant/Adopter must be 20 years of age or older.
All dogs currently residing in your home must be spayed/neutered and up-to-date on vaccinations.
If you are not selected to adopt your chosen dog, please note we do keep all applications on file. In the event you would like to be considered for another
dog please send us an email.
Your adoption fee will cover up to date vaccinations, deworm and heartworm/flea/tick prevention until adoption finalization.
PLEASE READ: If you are adopting a pet below the age of sexual maturity you will have to sign a spay/neuter contract to insure the puppy is altered at the date given by Pawsitive Hearts of Perry County. We do require proof to be sent to us of completed alternation via vet bill, vet contacting us, etc. If alteration does not take place by date given that is considered a breach in contract and we have the right to take ownership back of pet and/or pursue legal action.

Dogs/Puppies: $100
The following statement is required by Alabama law: STERILIZATION OF ANY ADOPTED ANIMAL IS REQUIRED BY 2022 Code ofAlabamaTitle 3 - Animals.Chapter 9 - Sterilization of Dogs and Cats. THE ADOPTER AGREES TO ASSUME FULL RESPONSIBILITY FOR COMPLIANCE WITH THE PROVISIONS OF Ala. Code § 3-9-1 WHICH PERTAIN TO THE TIMELY STERILIZATION OF THE ADOPTED PET. A PERSON WHO VIOLATES THIS ARTICLE IS SUBJECT TO CIVIL PENALTY. THE NEW OWNER MAY BE COMPELLED TO COMPLY WITH THE PROVISIONS OF THIS ARTICLE.
It not already done, I will have the dog spayed/neutered by 6 months and will immediately forward proof to the PHPC office.
Failure to comply with this requirement will result in the immediate return of the dog to Pawsitive Hearts of Perry County.
