Pledge and Policies:

This agreement (“Agreement”) is entered into as of {date} (“Effective Date”) between Olga Brouwer ("Coach") and {name} (“Client”), {name}, {address39} {address40}, {address41} {address},
- The time agreement is a total of 1 Hour Live Sessions, to be performed by May 14.
  
  This includes:1-hour Live 1:1 Coaching Session via Zoom
- Olga Brouwer promises the above named client that all information provided will be kept as confidential as the laws permit. I respect your willingness to be truthful, and I will treat this information as a special confidence.
- You can count on me to be accepting and supportive; asking questions and making requests. The main purposes of our interaction is to:
  
  ●      Remind you of your connection with your infinite resourcefulness.
  
  ●      Facilitate your self-discovery of your values and life purpose.
  
  ●      Support you in being aware of how and when your beliefs, commitments and actions align with your values and life purpose.
- Olga Brouwer reserves the right to anonymously use data and stories for marketing and presentation purposes.
- The term of this Agreement shall commence and end with one pro bono session before May 14, 2025. No exchange of funds is expected.
- If Olga Brouwer does not meet for a scheduled session due to illness or emergency, Olga Brouwer will make every effort to reschedule the missed session within thirty (30) days, on a day and time agreeable to both parties.
- Provider will make every effort to notify clients at least one (1) week in advance of coaching absences.
- Olga Brouwer’s total and cumulative liability for direct damages arising out of or in connection with this Agreement shall in no event exceed the fees paid by Client to Provider. In no event will Provider be liable for any indirect, incidental, consequential, special or punitive damages even if it has been advised of the possibility of such damages.

Life and Leadership Coaching for {name}

To begin our coaching relationship, I make the following requests. (Please check those you accept )

1. Please make the appointment on time. If for any reason you must be late, please contact me ahead of time.2. If you must change an appointment, please give me 24-hour notice and I will reschedule the appointment. If less than a 24-hour notice is given, a makeup appointment will be scheduled if feasible with my week's schedule.3. My promise to you is to be unconditionally constructive. Do you give me full permission to be bold and forthright in coaching you?4. Do you give me permission to interrupt you during long stories?5. Having coaching work for you is a shared responsibility. I request that you be responsible for having coaching work for you. You lead the session, ask for what you want, and let me know what is working and not working. This is about trust and self-discovery.6. I give permission to include me on your professional coaching hour log for certification or re-certification purposes. (Only my name, number of hours coached and email will be provided.)7. Be honest. As your coach, it is essential for me to be aware of past & present events, addictions, conditions, medications, and/or ways of thinking that my influence your life now. (i.e. Are you in a 12-step program? In therapy? Do you have a learning disability?)
