Annual Member Leadership Award Application Form

 

This award is presented at the OASIS Conference on 1st May at Blue Mountain. The objective of this award is to recognise and honour an OASIS member organisation that:

a) That demonstrates excellence, innovation and leadership in the developmental services sector and best exemplifies the interests and mission of OASIS

b) And that demonstrates collaboration, partnership, and professionalism and in so doing has provided an inspiration to others; and

c) And that, by a combination of perseverance, determination and positive attitude has assisted OASIS in achieving the goals inherent in its Mission Statement.

OASIS Mission Statement:
OASIS provides leadership through sharing ideas, information, and knowledge; and interacts with government and other organisations on issues affecting its members.

Recognition:
The Award will be in the form of a donation to the member organisation for an amount of one thousand dollars ($1,000.00).

Please complete the attached Nomination Form and submit it  by 18th April 2025.

NOMINATION PROCESS:

 

1. The Nomination Form must be completed by the person making the nomination.  You can nominate your own agency or another OASIS member agency that you feel is deserving of this award.

 

2. Nominations will be accepted until 18th April 2025.

 

3. The decision on the successful application will be final and will not be made known until the award is presented at the OASIS Conference at Blue Mountain from 30th April -2nd May 2025; unless to ensure the attendance of the representative of the organisation needs to be notified in confidence prior to the AGM.

 

4. It shall be the responsibility of OASIS to ensure that a cheque is presented to the appropriate agency along with the award to the recipient.
