2025 Cast Dance Academy Recital Order Form

- Please pre-order your package at least 24hrs before photo date. No same day orders will be taken.

- All dancers will be in the group photo. Those that are ordering a photo package will do individual photos in that costume.

- Please be early! We will send a schedule out a few days before with your times.

1 Costume package1 Costume. This includes 2 individual poses in the same costume as well as the group photo from the same dance and costume. If you have more than one costume you want photographed, please order one package for each costume. These are digital images and will be delivered via email no more than 3 weeks after the photo shoot.

Unlimited Costumes All costumes. This package includes all the dances your one dancer is in. 2 solo photos and the group photo from each dance. These are digital images and will be delivered via email no more than 3 weeks after the photo shoot. Please list all your dancers dances and remind them to stay after each group photo to do their 2 poses.

Group Photo 1 Group photo. This is a digital images and will be delivered via email no more than 3 weeks after the photo shoot. Please write the name of the dance and call time on the form.
