IMPORTANT -- PLEASE READ ALL INSTRUCTIONS!

One form per dog per class. If you wish to enroll in 2 classes with the same dog, you must fill out this form for each class.

CURRENTLY NO NEW STUDENTS ARE ACCEPTED FOR AGILITY CLASSES.

All new students MUST email the dog's vaccine records to the registrar at hamiltondogclub@gmail.com -- or text to 513-760-3093.

Note: Under "Terms", you are required to scroll thru and read all of the terms before you will be able to click on the "Accept" button.

After you have filled out the form completely, choose your payment method (check or paypal), then click on the submit button to complete this registration.

NOTE: After hitting the paypal button, your system may show a blue circle as it "thinks". Check your email before entering this form again!

Use coupon code RESCUE if you are signing up for Puppy or Level 1 Basic Obedience, only IF you obtained your dog from Animal Friends Humane Society or Animal Adoption Foundation (MUST SHOW PROOF of adoption).
