As a reminder, LIFEMAPP is a web-app designed to facilitate and complement the work of clinicians in welcoming and supporting patients and their families. It provides them with a tool containing essential information and guidance to help them "familiarize" themselves with the ICU environment and prepare for a return to normal life.

Who are we?
The One O One (101) Endowment Fund is a non-profit organization dedicated to advancing the field of intensive care by funding training programs for healthcare professionals, research projects, and initiatives to better support patients and their families during and after their stay in the ICU.

Why this form?
In this form, we will ask for all the necessary information to reference your unit on LIFEMAPP. This information will be used to create a dedicated page for your unit on lifemapp.one-o-one.eu and to ensure effective communication between your unit and the 101 Fund.

Data Usage
The 101 Fund is strongly committed to data protection. By clicking "Next," you consent to the use of your responses by the 101 Fund as part of the LIFEMAPP program.

The collected data will never be shared outside the 101 Fund.

The information provided below will be strictly used for communication between your ICU department and the 101 Fund.

It will not appear on your ICU page.

The information provided below will be used to reference your ICU on LifeMapp and will be displayed on your dedicated ICU page.

We encourage you to open this example page alongside completing the form so that you can visualize the final result.

 

If you wish, you can add additional messages to your ICU page (e.g., message from the psychologist, etc.).

To guide families viewing your ICU page, you can also indicate the most suitable time slots for visits, phone calls, and medical consultations.

Please note that these time slots are provided for reference only and may vary depending on the ICU’s activity and emergencies or be adjusted to better accommodate your constraints.

Once your ICU is listed on LifeMapp, we will send you posters with a QR code linking directly to your ICU's page.

LifeMapp posters are an important tool to encourage patients and their loved ones to visit your ICU's page and use this resource designed to support them throughout their ICU journey. We strongly encourage you to maximize the visibility of these posters by placing them at the reception desk, in hallways, waiting rooms, bulletin boards, post-ICU consultation rooms, etc.

The information you provide below will not be displayed on your ICU's LifeMapp page and will not be shared outside the 101 Fund under any circumstances.
