Purpose: The Florida Assisted Living Association (FALA) Annual Awards were created to recognize Facility Member employees and volunteers who have made a positive impact on the operations of their Assisted Living Facility (ALF) or Adult Family Care Home (AFCH) and the lives of their residents, and Associate Member businesses who have been long-term and active FALA members and supportive of FALA Facility Members.

Who can I nominate?  Criteria for each award is stated below. Based on award criteria, nominees may include ALF administrators; AFCH owners; facility employees who go beyond the call of duty to bring positivity, smiles, comfort, cheer, etc. to your daily operations and your residents; facility volunteers who unselfishly give of themselves; or an Associate Member business who has positively supported the operations of your ALF or AFCH. Only FALA Facility Member employees (Assisted Living Facility or Adult Family Care Home) or FALA Associate Member businesses are eligible.

What should I do?  Complete and submit the nomination form below by BLANK.  If you have more than one nominee, you must submit a new nomination for each. 

When will award recipients be selected and recognized?  Award recipients will be selected by the FALA Awards Committee once all nominations have been submitted and reviewed. Selected award recipients will be recognized at the FALA Annual Awards Dinner on Wednesday, July 30th, in Orlando, held in conjunction with the FALA Annual Conference & Tradeshow.  You will be contacted after the Awards Committee votes to advise if your nominee was selected; and if your nominee is selected, you will be asked to help coordinate their attendance at the Awards Dinner. If an award recipient is unable to attend in person, other arrangements can be made. Attendance at the Awards Dinner will be complimentary for the award recipient and one guest; however travel and hotel expenses are not covered. Tickets for additional guests and colleagues will be available for purchase upon request.

• ALF Administrator of the Year Award- This award recognizes an outstanding individiual in the ALF Administrator profession who meets the following guidelines: Has been active in ALF administration for a minimum of three (3) years. Has current, direct supervisory responsibility. Demonstrates outstanding leadership ability. Has contributed significantly to their profession. Has shown true leadership qualities in his/her facility and is a trendsetter for other administrators. Has shown exceptional efforts to increase the quality of life in their assisted living facility. Has a commitment to earn and extends respect to residents and colleagues.
• Adult Family Care Home Provider of the Year Award- This award recognizes an outstanding Adult Family Care Home provider who: Has been a licensed Adult Family Care Home provider for a minimum of three (3) years. Has shown tremendous leadership and is highly respected by his/her colleagues. Ensures the safety and wellbeing of his/her residents and makes them feel like part of the family. Serves as a mentor to other adult family care home providers.
• Activity Engagement Award- This award recognizes activities engagement staff who are motivators, creators, or innovators, and who provide out-of-the-box activities to their community's residents.
• Compassionate Care Award- This award recognizes employees who demonstrate an inner spirit of love, care, and empathy for not only the residents but for everyone he/she meets.
• Culinary Creation Award- This award recognizes dining service staff who present meals that are beautifully displayed and taste even better, as well as having residents and visitors eagerly awaiting mealtime.
• Maintenance in Excellence Award- This award recognizes maintenance staff who ensure that the upkeep of their facility's building and grounds are always in pristine condition.
• Hero of Hope Award- This award recognizes staff who are shining stars and always provide positivity to make others' dim lights shine brighter.
• Volunteer Award- This award recognizes individuals who unselfishly volunteer their time without expecting recognition.
• Associate Member of the Year Award- This award recognizes businesses who have been long-time members and supporters of the Association and who meet the following guidelines: Has been a FALA Associate Member for at least five (5) continuous years. Has been actively involved in FALA events through participation in the annual conference, chapter meetings, Legislative Days, and/or other events. Has provided financial support to the Association through programs such as being a FALA Palladium Partner, conference and/or event sponsorships, etc.  Has demonstrated flexibility to meet the needs of FALA membership, a willingness to adjust service fees to FALA members, and ensuring that goods and services are timely delivered to members.
