Student Code of Conduct
Failure to adhere to this set of guidelines at any time throughout the school year will result in possible dismissal from ARROWS. Please thoughtfully and carefully read these guidelines and have your student sign in agreement.

Please note: In the best interest of everyone involved, we always begin new enrollment with a one-month “probationary period.” During this time, our teachers and leadership will decide if ARROWS is a good fit for the student.

ARROWS Collaborative Photo Release Waiver
I, Parent/Legal Guardian Name* hereby grant and authorize ARROWS Collaborative consent to take, edit, copy, publish, distribute and make use of any and all pictures or video taken of my minor child(ren), to be used in and/or for legally promotional materials including, but not limited to newsletters, flyers, posters, brochures, advertisements, fundraising letters and submissions to journalists, websites, social media sites and other print and digital communications, without payment or any other consideration.

This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing.

I understand and agree that these materials shall become the property of ARROWS
Collaborative and will not be returned.

I hereby hold harmless, and release ARROWS Collaborative from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.

I warrant that I am the parent/ legal guardian and have read this release before signing below and I fully under stand the contents, meaning and impact of this release.

ARROWS Collaborative Release of Liability
ACTIVITY RELEASE OF LIABILITY

(READ CAREFULLY - THIS AFFECTS YOUR LEGAL RIGHTS)

In exchange for participation in this homeschool enrichment program, organized by ARROWS Collaborative, and the use of the property and facilities at St. Andrew Presbyterian, (201 RM 1431, Marble Falls, TX 78654), I agree for myself and for the members of my family, to the following:

1. I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by ARROWS Collaborative or the tutors, employees, representatives or agents of ARROWS Collaborative.

2. I recognize that there are certain inherent risks associated with the above-described activities, and I assume full responsibility for personal injury to myself and to my family members, and further release and discharge ARROWS Collaborative and ARROWS Collaborative tutors and volunteers for injury, loss, or damage arising out of my or my family's use of or presence upon the facilities used
by ARROWS Collaborative, whether caused by the fault of myself, my family, ARROWS Collaborative, or other third parties.

3. I recognize that ARROWS Collaborative is NOT an allergen-free facility and I assume full responsibility for any personal injury to myself and my family members and further release and discharge ARROWS Collaborative and ARROWS Collaborative tutors and volunteers of any responsibilty for any purposeful or accidental ingestion or exposure to a possible harmful allergen.

4. I agree to indemnify and defend ARROWS Collaborative against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family's use of or presence upon the facilities of ARROWS Collaborative.

5. I agree to pay for all damages to the facilities rented, used, or owned by ARROWS Collaborative caused by my or my family's negligent, reckless, or willful actions.

6. Any legal or equitable claim that may arise from participation in the above shall be resolved under Texas law.
