    Use this form to apply to obtain services from the Office of Accommodations for the academic year.
  • Mission of the Office of Accommodations:

    The mission of the Office of Accommodations is to support eligible students with reasonable and appropriate accommodations and to ensure equal access to programs and services that enhance student learning and development.Qualified persons with disabilities must meet the Warner University regular admissions standards. However, admission eligibility will be considered on a case-by-case basis to afford applicants an equal educational opportunity. Qualified applicants with disabilities will not be denied admission solely on the basis of their disability. It is not necessary, nor recommended, that students disclose their disability in the application process. Once admitted to the University, students with disabilities requesting accommodation should contact the Director of the Office of Accommodations to register for services.A person is eligible for services if he/she is otherwise qualified for the program(s) is a person with a disability as defined by ADA and Rehabilitation Act 504 has identified himself/herself to Disability Services by completing an application for accommodations, and has presented appropriate documentation regarding the disability to the Office of Accommodations.Note: After documentation has been reviewed and approved by the Director of the Office of Accommodations, an accommodation plan is developed. Accommodations are effective after the accommodation plan is finalized. Accommodations are not retroactive. Also, documentation required by the Office of Accommodations may not be sufficient at other universities or testing agencies.
  • Documentation Guidelines:

    Documentation must be typed on a professional letterhead and include the name, title, professional credentials of the evaluator (including license or certification and area(s) of specialization), place of employment, state in which the individual practices, and the evaluator’s signature.Evaluations must be performed by a professional who is licensed or certified to evaluate and diagnose the particular disability.The professional conducting the assessment must be impartial and cannot be a family member.An Individualized Education Plan (IEP) or 504 may not by itself be sufficient documentation to determine eligibility.Students are responsible for all costs associated with obtaining reports, examinations, testing, etc.Because accommodations are not retroactive, students requesting accommodations for the first time should submit their applications and documentation in a timely fashion so that the University has ample time to review these materials, determine eligibility, and make necessary arrangements.Documentation must clearly indicate that the condition results in substantial limitations in major life activities applicable to our academic environment.
  • Application for Accommodations

    This application will be reviewed by the Director of the Office of Accommodations for determination of eligibility for services requested. All communication will be directed to the student listed on the application. Please be sure your student checks his/her email.
  • Supporting Documents

    Please upload any documents that you would like us to review along with your application. If you are unsure if a document meets the guildelines for establishing eligibility, please refer to our website and review the document guidelines listed there. The Director of the Office of Accommodations will review this application and all supporting materials in an effort to establish eligibility for services. The Director may reach out via email to the student listed on the application for clarification or discussion of a request, to request  additional documentation, or to schedule an application review for approval.
