    ✅ Please provide your DEA registration information and Professional Liability Insurance information and upload the following required documents:

  • 📂 4️⃣ Mileage & Earnings Tracking Sheets

    We have created a unique Mileage and Earnings Tracking sheet to help you with tax filing. This will be sent to your registered email so please check your inbox.
  • 📝 Gusto Self-Onboarding: Direct Deposit & Form I-9

  • To ensure you get paid on time and everything is set up properly, you should receive within 48 hours an email from Gusto, our payment platform, with a link to complete your onboarding.

    What you’ll need to do:

    1. Click the link in the Gusto email.


    2. Fill out your tax form (Form W-9).


    3. Enter your contact and payment details (bank info for direct deposit).


    4. Submit everything securely through Gusto.

  • For any urgent concerns, such as unforeseen cancellation of shifts, emergencies, or to confirm a relief schedule, please contact us via our Google Voice SMS line. We have DVM staff from the Philippines on standby to assist you 24/7. If you have overtime in a shift, please text by the end of day so we can modify your payment. If you have any suggestions to make our service more convenient for you, please reach out.
