    Here are a few important steps to ensure smooth communication and timely support for you during your relief work.

  • If you’re interested in working shifts at Banfield locations, please also sign the Banfield Services Agreement below. Review the agreement carefully and complete all required fields.     Please provide at least one reference from a previous hospital where you were employed. Ideally, a practice manager or supervising veterinarian who can speak to your clinical skills and professionalism.

  • 6️⃣ of 8️⃣: Mileage & Earnings Tracking Sheets

    We have created a unique Mileage and Earnings Tracking sheet to help you with tax filing. This will be sent to your registered email so please check your inbox.
  • 7️⃣ of 8️⃣: Gusto Self-Onboarding: Direct Deposit & Form W-9

  • Gusto is Uplift.Vet's payment system. To ensure you get paid on time and everything is set up properly, you should receive within 48 hours an email from Gusto with a link to complete your onboarding.

     

  • Here’s what you’ll need to do on Gusto as soon as you receive the self-onboarding link. Please complete at least 7 days before your first shift:

    If as an Individual

    1. Click the link in the Gusto email.
    2. Fill out your tax form (Form W-9). For individuals on a 1099, you will receive these tax forms in January of each year.
    3. Enter your contact and payment details (bank info for direct deposit).
    4. Submit everything securely through Gusto.

    If as a Business

    1. Click the link in the Gusto email.
    2. Fill out your tax form (Form W-9). Check “S-corporation” as federal tax classification on Section 3a. 
    3. Enter your corporation’s contact info, EIN and payment details (bank info for direct deposit). The IRS does not require end of year tax statements to be sent to business.
    4. Submit everything securely through Gusto.
  • 8️⃣ of 8️⃣: Please list the individual who referred you. We are only taking applications from direct referrals.

  • 📞 Additional Information & Support

  • For any urgent concerns, such as unforeseen cancellation of shifts, emergencies, or to confirm a relief schedule, please contact us via our Google Voice SMS line. We have DVM staff from the Philippines on standby to assist you 24/7. If you have overtime in a shift, please text by the end of day so we can modify your payment. If you have any suggestions to make our service more convenient for you, please reach out.

  • ⚠️ Wait! Have you signed the Banfield Agreement?

    If you plan to take shifts at Banfield Pet Hospital and have not yet signed the embedded document, please go back to Part 2 of 8 to complete it now. Your application cannot be processed for Banfield shifts without a signature.
