Relief Angels Pet Adoption Application
Adoption Fee $300 for dogs. A voucher coupon is emailed to the adopter for $150 if the puppy is too young to have been altered yet. Cats already altered are $125. Kittens are $150 with a $75 voucher. (These fees covers vaccinations and all medical care as up to date as age appropriate and time permitting, deworming and a microchip).

Transport fee for out-of-area adopters: $75 (We will transport up to 150 miles.)

Sumitting this application does not mean you are commiting to a pet, nor does it guarantee you will get that pet. It is simply a tool we use to help both you and us identify a good potential match for your family and lifestyle.

I understand that completing this application does not guarantee I will be allowed to adopt a pet, and that the application is a tool to help Relief Angels determine if this pet and my family are a good fit for each other. I acknowledge that all information provided on this form is true and correct. I understand that any misrepresentations may result in my not being approved and/or removal of the adopted dog from my possession. I also certify that I have never been convicted of animal cruelty or neglect.

I agree to allow a representative from Relief Angels to contact my vet for a reference and also agree to allowing a home visit prior to and/or shortly after adopting this dog. If I am approved to adopt a dog, I agree to pay a non-refundable adoption fee and hand sign an adoption contract at the time of adoption.

I understand that all dogs adopted from Relief Angels will be vaccinated as age appropriate as possible prior to adoption. I also understand that Relief Angels will only Spay/neuter pets after they are old enough for the health and safety of the pet.
