                             Behavioral Aid

    Addressing behavior management safety needs, supporting emotional and social needs, giving academic support, and implementing behavioral improvement plans

    •  Must have a Bachelor's Degree in Human Service Field or High School Diploma with 5 years human experience required
    •  High Level 24$ hour
    •  Moderate level 18$ an hour

    Mental Health Counselor/Therapist

    Establishes positive, trusting rapport with patients. Diagnoses and treats mental health disorders. Creates individualized treatment plans according to patient needs and circumstances. Meets with patients regularly to provide counseling, treatment and adjust treatment plans.

    •  Masters/Doctoral degree in either counseling, psychology, social work, manage and family therapy or similar field
    • Licensed Counselor or Social Worker with one of the following licenses: LPC, LCSW, LMSW, associate.
    • Licensed or license eligible with minimum of 5-year post-graduate experience.
    • Previous experience providing clinical assessment & therapy services for Department of Family and Children Services (DFCS) is a plus
    • High Level-35$ an hour
    • Moderate level -30$ an hour

          Transporter

    • High School Diploma or GED required.
    • Minimum of one year of working within the human services field, providing transport services via Department of Family and Children Services (DFCS)
    • Good Driving Record - will request Motor Vehicle Report (MVR)
    • Reliable transportation
    • Experience with writing progress notes in the BIRP format
    • Efficient with strong attention to detail
    • Must have High School Diploma
    • Must have full coverage insurance
    • 15$an hour/ .40cent mileage reimbursement.

     

                   Drug Screen Collector

    Provide drug screen collections. Travel to the patient's home to perform a urine, hair, and/or saliva specimen collection as requested by the DFCS Case Manager. Other duties may be assigned. Contribute to Georgia HOPE's culture of safety and inclusion by demonstrating a commitment to laming how to engage with issues of diversity, equity, and inclusion.

    • Hair Follicle, Urine Analysis, Oral Swab- Quest Diagnostics
    • Fingernail test- Expert Tox

    • All test and results are due back within 48 hours.

            Emergency Placement

     24-hour supervision for displaced foster children.

  • This is an agreement between Building Bridges BetweenFamilies LLC/Agency/Dionna Lakes and /Subcontractor/   *   *    This agreement was reviewed and entered as of    Pick a Date*  . This agreement is governed by the laws of the state of Georgia.
     
    The Agency & Subcontractor agree to adhere to the documented guidelines for performance and compensation as set forth below:
    The subcontractor agrees to take on a contractual position as a    . The subcontractor agrees to perform all work in a professional and ethical manner, upholding the mission and core values of the agency. The subcontractor agrees to maintain all required      credentials. The subcontractor agrees to be available to the agency via phone and email, within standard business hours of Monday- Friday, 9am-5pm. Evening & Weekend hours will be utilized at the discretion of Building Bridges Between Families LLC. (Subject to Vary)
    Duties and Responsibilities
    • Case Note Submittal &Approval
    o Submit all Subcontractor case notes within 48 hours of visit.
    o Utilize the correct note template for service provided.
    o Ensure all notes discuss: Behaviors, Interventions, Responses, and Future Case Goals
    o Ensure all case notes have correct and complete billing codes.
    o Ensure all notes have correct time, place, and people serviced.
    o Ensure all notes are submitted to YAHOO account within 48 hours of visit.
    o Ensure timely submission of all notes to Argo inaccordance with agency policy of 72hr.
    o Ensure Subcontractor invoice logs are submitted by last day of month. 
    • Mileage Logs Submission
    o Complete and submit all Subcontractor mileage logs after each visit.
    o Ensure all fields on mileage logs are complete. Only field left blank on mileage log will be invoice number.
    o Ensure all clients serviced are listed
    o Ensure that the purpose of trip is listed for each leg
    o Ensure you are using the address (yours/ agency) that isclosest to the location. 
    o Ensure all mileage logs date and miles match the case note for that date.
    o Ensure all mileage logs are signed.
    o Ensure timely submission of mileage log by the last day of the month.
     
    • Subcontractor Invoice Log
     
    • Ensure all Subcontractor invoice logs match your case note
    • Ensure all Subcontractor invoice logs are billed correctly
    • Ensure all Subcontractor invoice logs are submitted by the last day of each month
    Billing Submission Date
    o Ensure all case notes are completed, correct, and submitted to billing by the last day of the month
    o Ensure all Subcontractor invoices are submitted by the last date of the month
    o Ensure all mileage logs are submitted by the last day of the month
    o Ensure all receipts are submitted with 48 hours of the visit
    o Ensure all state corrections are resubmitted within 24 hours
    o Any mileage logs or case notes over billed will be removed from the Subcontractor next payroll
    Service Authorizations
    o Input all notes in Argo under the correct SA number.
    • Ensure each SA covers the date of service listed on the case notes
    • Ensure each SA has units that covers and supports services listed on case notes
    • Ensure you are utilizing and implementing the correct SA number on mileage logs and case notes.
    • Ensure SA does not expire
     
    Visits Expectations
               
    • Subcontractor must ensure their badge is always visible.
    • Subcontractor must always be dressed appropriately. Clothing unaccepted, but not limited too are: gym shorts, sweat pants, tank tops, clothing with foul/offensive language, etc.
    • Subcontractor is not allowed to smoke any tobacco/vapes or consume any alcohol during the time of service.
    • Subcontractor must confirm all visits within 24 hours of the expected service date.
    • Subcontractor must be on time for all pickups, drop offs, and scheduled meeting place.
    • Subcontractor must cancel visit before 24 hours of the expected visit time. Must be approved by Director
    • Subcontractor must monitor the visit from no more than 3 ft away from each party.
    • Subcontractor must intervene if any situation arise that may cause harm to the child, this includes but not limited too: verbal altercations, false promises, offensive language.
    • Subcontractor will not allow the child to be left alone with anyone for anything at ANY TIME!
    • Subcontractor must complete a note and mileage log, and submit all receipts after each visit.
    • Subcontractor must report any visit concerns with the DFCS Case Manager within 24hrs. of the visit.
    • Subcontractor must report all reports of child abuse witnessed or disclosed during a visit to 1 800 GA CHILD(422-4453)
     
      Company Email:
    • Each subcontractor will be assigned a company email account.
    • Each subcontractor must create a password for their account.
    • Company emails need to be utilized for communication between DFCS, Foster parents, courts, or any service providers.
    • Company emails need to be monitored daily.
    • Emails sent to the company email must be responded to within 48 hours of receipt.
     
    Monthly Agency Meetings
    • Each Subcontractor must attend monthly agency meetings on the 15th of each month. If the 15th falls on a weekend, monthly meetings will be held on the business day before.
    • Each Subcontractor must discuss all case concerns during this meeting.
    • Each Subcontractor must be prepared to lead the meeting each week. (Will be trained)
     
     
    ​
    Rates & Compensation
    • ​Behavioral Aide: $18/hr. to $24hr
    • Emergency Supervision: $22/hr.
    • Transporter/ Escort: $15/hr.
    • Mental Health Therapist BS: $30/hr.
    • Mental Health Therapist MS: $35/hr.
    • Mileage Reimbursement: $.50cent a mile
    • Drug Screen Collector: $50/ each test
    o Each payroll will cover all work performed from the previous month 1st- 30th.
    o Paychecks will be distributed monthly on the 15th of every month.
    o Any mileage logs or case notes over billed will be removed from the Subcontractor next payroll
     
    • Disciplinary Consequences
    ​Verbal Warning- Director will verbally coach the Subcontractor on their disciplinary action. Verbal warning examples include but not limited too; cancelling cases, note submissions, errors on mileage logs, etc.
    ​Corrective Action Plan- Director will implement a Corrective Action Plan to address the Subcontractorbehavior. The plan will list steps and preventive measures for the Subcontractor to ensure they are aware of areas needed for improvement. Correction Action Plans examples include but not limited too; constantly submitting late notes, violation of any company policy, late Argo submission, etc.
    ​Grounds for Immediate Termination- Director will provide all Subcontractors terminated with a separation notice that specifies their offense. Termination is not eligible for appeal. Immediate Termination examples include but not limited too, Committing Fraud, leaving children unsupervised, receiving a CPS case, etc. Director can and will terminate Subcontractor at their discretion.
     
    Termination of Agreement
    • This agreement will become effective on the datesigned until the services provided for herein have been performed for a period of this contract unless sooner terminated. Early termination of this contract by Subcontractor shall be made effective with 30 daysadvance written notice.
    • The agency has the right to terminate this agreement at any time. If the agency exercises its right to terminate the agreement any obligation it may other wise have under this agreement shall cease immediately, except that the agency shall be obligated to compensate contractor for all work performed up until the time of termination. Final paycheck will be directly deposited or mailed to the contract on the following pay date after termination.
    ■ ​If the subcontractor exercises its right to terminate the agreement, any client obligations will be not terminated until obligations are met. All confidentialinformation will be disposed of in accordance with agency policy. All communication on behalf of the agency, with agency employed, and with the agency's clientele will cease upon final termination date.
    • ​Renewal of Agreement - This agreement will be reviewed for renewal 60 days before the end date of this contract. During that time compensation and services will be revised accordingly.
    • ​Modifications - Modifications to this agreementmust be made in writing and will be effective only if reviewed, agreed, and signed by both parties.
