- Serious injuries (e.g., fractures, dislocations, concussions, dental injuries)
- Life-threatening events (e.g., cardiac incidents, head trauma)
- Exposure to communicable diseases (e.g., COVID-19, influenza, meningitis)
The Participant and Parent/Guardian confirm that:
- They understand the nature and physical demands of camp activities
- They voluntarily assume all associated risks
- They acknowledge that V3 Basketball Association, including its staff, directors, officers, agents, volunteers, beneficiaries, successors, and assigns, cannot be held liable for any injuries, illnesses, or exposures as outlined in items 1–4 above

Acknowledgment of Understanding
Participant and Parent/Guardian have read this agreement to participate and fully understand its terms. Participant and Parent/Guardian acknowledge freely and voluntarily signing the agreement to participate and intend the signatures to signify a complete assumption of the inherent risks of participating in or observing activities at V3 to the fullest extent permitted under Ontario law.
In signing this assumption of risk agreement, I acknowledge that I am consenting to the Participant’s participation at V3 and acknowledge that the Participant and Parent/Guardian expressly assume all inherent risks of participating in a V3 activity.
Waiver of Liability
In consideration of V3 Basketball Association accepting the enrollment of the Participant in a program at V3 and/or Participant’s use (today and on all future dates) of the property, facilities, parking lot, buildings, fields, equipment, housing, dining areas (if applicable) and any and all other services provided by V3, the Participant and Parent/Guardian, on behalf of the Participant’s heirs, next of kin, personal representatives, and/or assigns, shall not sue or bring any action against V3 Basketball Association, its affiliated companies, or any of its members, directors, officers, employees, volunteers, sponsors, agents or contractors, and shall release each of them from liability in connection with all claims for:

Indemnification
Participant and Parent/Guardian agree to indemnify, defend and hold harmless V3 Basketball Association, its affiliated companies and each of its members, directors, officers, employees, volunteers, sponsors, independent contractors, and agents, from any and all claims or liability of whatsoever kind or nature, including but not limited to personal injury, including loss of life, or property damage, court costs, and/or attorney's fees, which may arise out of, or result from:
- Participant’s enrollment in a V3 program
- Receipt of medical care or treatment for any physical or mental condition
- Use of or presence at V3’s facilities, services, premises and equipment

Severability and Jurisdiction
Participant and Parent/Guardian expressly agree that this waiver is intended to be as broad and inclusive as is permitted by the law of the Province of Ontario, and that if any portion thereof is held invalid, the remaining portion of the waiver will continue in full legal force and effect. Participant and Parent/Guardian also agree that all disputes shall be resolved using binding arbitration at an office in the City of Toronto, Ontario.
Acknowledgment of Understanding
Participant and Parent/Guardian have read this waiver and fully understand its terms. Participant and Parent/Guardian understand that the Participant is giving up certain rights, including the right to compensation for injury. Participant and Parent/Guardian acknowledge that they are signing the agreement freely and voluntarily, and intend their signatures to be a complete and unconditional release of all liability to the greatest extent allowed by law. Parent/Guardian acknowledges that they are consenting to the Participant’s participation in a program at V3 and understand that all risks are expressly assumed by the Participant and themselves and all related claims are expressly waived in advance.
