PROCESS FOR BECOMING AN APPROVED PROVIDER
Prospective Continuing Education (CE) providers must complete and submit this application 30 days prior to their scheduled program. Please read the following guidelines for provider applications.
1. The program must fulfill the following requirements:
- The program must be a minimum of one (1) hour.
- Speakers must have the background and experience necessary to teach the course.
- Abbreviated CVs are required for each presenter.
- The program should have qualified instruction and direction to assure that the educational objectives will be fulfilled.
- Program content must be a formal scientific/career-related continuing education program.
- The program content, format, and methodology must be conducive to achieving the program objectives.
- Learning objectives should be stated in terms of what the participant will be able to do at the end of the program that he/she could not be expected to do prior to the program. Each objective should start with an action word like discover, utlize, implement, etc.
- The program should be a planned learning experience designed to promote the continual development of knowledge, skills, and attitudes.
2. Program approval and the number of CEC's and contact hours earned will be determined by ACHI and reported to the provider.
3. Once approved, the provider should advertise that the program has been approved for ACHI Continuing Education credit.
4. The provider must provide to each participant, as appropriate documentation, a "Certificate of Attendance" which includes the following (may be completed after the program):
- the number of contact hours and CEC's earned;
- the ACHI name and address;
- the course number issued by ACHI;
- a selection of categories and/or topics for the continuing education activity.
5. An attendance roster that lists all of the program participants must be sent to ACHI in the required Micorsoft Excel format. Click here to download a copy of the ACHI-approved Roster Attendance Sheet. (PDFs, jpegs, pngs, and other file formats will not be accepted.)
6. If approved program content is revised after approval, an updated CE activity form must be submitted to ACHI prior to issuing certificates of attendance.
