Faith Formation Registration

Registration for the 2025-2026 year for Pre-K - 12th Grade Students. The entire family can use this form.
CGS Level 1 (age 3, must be potty trained) will be held on Tuesdays from 3 - 4 p.m..

CGS Level 1 (ages 3-9) will be held on either Monday or Tuesday from 5 - 6:30 p.m..

CGS Level 2 (ages 3-9) will be held on either Monday from 5 - 6:30 p.m.. Tuesday is currently full, please note if you would like to be added to a waitlist.

4th - 6th grade will be held on Tuesdays from 5-6:30 p.m. in the Holy Family Center.

7 - 12th grade students will join Emmaus Groups hosted on different nights. To be announced soon! (Based on student avaliblity and mentor avaliblity.)

Those seeking Confirmation in 8th grade year will meet with Makenzie to discern rediness before joining our 8 week course. All those seeking Confirmation must be involved in an Emmaus Group before starting the course.

Registration after June 30, 2025 is $50 for one student and $100 family max.

$40.00 for one child, $75.00 for two or more Maximum is $75.00 per family for early registration by June 30, 2025. After June 30, 2025 price increases to $50 per child and a maximum of $100 per family. If financial help is needed, please contact Tony Allen at 616-935-8737.
