PURCHASE ORDER

PO NUMBER WILL BE AUTO-ASSIGNED

Submit purchase orders for upcoming costs that will need to be paid out in the future. Once you complete this form, the PO number will be auto-assigned and emailed to you.

If you already have an invoice you need paid out, please submit a "Payment Request" instead.

PAYMENTS WILL NOT BE PROCESSED FROM PURCHASE ORDERS.

• Any and all domestic payees (businesses or individuals) must provide a W9.
• Any international payees working in the USA need to provide a W8-BEN if they are an individual or a W8-BEN-E if they are a company / registered entity. 
• International payees NOT working in the USA do NOT need to provide a W8.

Any individuals approved to be paid via A/P check for services rendered may be considered misclassified as a contractor by governmental entities (including labor boards in the jurisdiction they are working in). In such cases, applicable overtime and income taxes may be due after the fact as well as penalties and interest. Additionally, the individual may not be covered by workers compensation insurance.
