By completing this form, you are joining a vital community of NHS staff facilitating the Hospital Rooms Digital Art School nationwide. We ask that you watch our short training videos before you get started- 2 minute welcome, what's in the box and leading a workshop. As well as your art materials, you will receive access to training, resources, and our monthly facilitator forums to ensure you have everything you need to participate in and make the most of our monthly workshops. Please complete the form below to tell us more about you and your service. You will be invited to share this information each year, and your responses will help us to ensure that we can continue to deliver high-quality artist-led workshops which are accessible and impactful for your service users, and that we are supporting you to deliver them. The form should take 2-3 minutes to complete. Once this form is submitted, your Facilitator Certificate will be emailed to you within the week!
