Throughout the school year, we will be taking various field trips within our community and surrounding areas to help broaden your child’s learning experiences. Please complete the information below giving your child permission to participate.
The below named student has permission to participate in field trips associated with Faith Christian Academy. I also understand that by signing this form, my child has permission to be transported by the Faith Christian Academy school bus and/or vans to the field trips. Unless otherwise specified, your child will return to Faith Chrisitan Academy in time for pick-up at the end of the day.
While we at Faith Christian Academy will always take care of your child as if he or she were our own child, we do understand that unforeseen accidents are possible. By signing this form, I also understand that the staff, volunteers, and administration relinquish any personal liability and, therefore, the staff or school cannot be held responsible for unforeseen accidents.
