MINNESOTA SOLDIERS ATHLETIC DEVELOPMENT AGREEMENTS AND WAIVERS

1.I/we the parent/guardians of the above named candidate hereby give my/our player approval to participate in any and all MN Soldiers activities, including transportation to and from activities.

2. I/We know that participation in Football, Basketball, and/or cheerleading may result in serious injuries and protective equipment does not prevent all injuries to players and do hereby waive, release, absolve, indemnify, and agree to hold harmless Minnesota Soldiers Athletic Development Limited non-profit, the organizers, sponsors, supervisors, participants, and persons transporting my/our child to and from activities from any claim arising out of any injury to my/our child weather the result be of negligence or for any other cause.

3.I/We agree to return upon request the uniform and other equipment issued to my child in as good conditions as when received except for normal wear and tear.

4. I/We agree and acknowledge that staff and players of MN Soldiers Athletic Development will be engaging in a brief corporate prayer after practices and games, however this is not mandatory participation is at parents/guardians discretion.
