To register for the camp, you must complete the form in its entirety. Cash payment is accepted at the door, but if your plans change, PLEASE let us know as soon as possible. Please follow up with an email to mivbcoach@outlook.com to confirm registration. THERE IS CURRENTLY A WAITING LIST FOR THE AFTERNOON SESSION. WE ARE ADDING AN EVENING SESSION FOR 7TH-9TH GRADE. IF THAT IS AN OPTION FOR YOU, THERE WILL MOST LIKELY BE AN OPENING.

I hereby assume all risks associated with this event and I hereby waive, release, and discharge any and all claims for damages, personal injury of any kind which may hereafter occur to me or to any person as a result of my participation in this event. This waiver and release shall be binding on my heirs and assigns, and shall run in favor of Coach Patrick, MIHS Volleyball players/coaches, Merritt Island High School, Cape Coast Volleyball Club or anyone else connected with this event.
