Thank you for your interest in joining our orchestra family.

Before completing the registration form, please have the following information ready for your convenience:

- music teacher's name and email

- player's dietary restrictions, allergies and medical information

- a recent digital photo of player

- player's emergency contact (please note that this has to be somebody different from the primary contact who's filling in the form)
Event task force - We are looking for up to 20 families to form a consistent task force for the whole season. Selection based on past commitment, skill, availability and order of application.

Application for these positions will be available in late August.

RDYO Society Membership
With each registration, one adult will become a member of the Richmond Delta Youth Orchestra Society, a registered charity, and will be able to join and vote at society general meetings. The voting member must be over 19 years-of-age.

Consent Agreements and Waivers
Richmond Delta Youth Orchestra Society Consent

I consent to the Richmond Delta Youth Orchestra Society representatives collecting, using and disclosing my personal information including information obtained in the past and future, for the purpose of: processing grant applications, membership registration and tuition fees; membership and orchestra players lists; scholarship and Friends of the Orchestra donations; concert programs, volunteer management.

I have read the Richmond Delta Youth Orchestra Society's Members Consent to Collect, Use and Disclose Personal Information statement above, and agree with the statements provided. This consent is limited to the current registration term and is renewed annually.
Liability Waiver

In consideration of the Richmond Delta Youth Orchestra Society providing continuing membership and services to me, the player, and other good and valuable considerations, I, the player, and my parent/guardian, including representative heirs, executors, administrators, successors, assigns, committees, trustees and agents (as the case may be), HEREBY WAIVE, REMISE, RELEASE, AND FOREVER DISCHARGE the RDYO from any and all manner of actions, causes of action, suits, proceedings, liabilities, obligations, debts, contracts, claims and demands or damage of any nature or kind whatsoever arising from my attendance at and participation in any rehearsal and camp activity. In this release, the “RDYO” includes the Richmond Delta Youth Orchestra Society and its respective directors, officers, employees/contractors, agents, volunteer assistants, predecessors, successors, assigns, liquidators, receivers, managers and trustees.
Youth Media Consent

I hereby authorize any images, audio, or video footage taken of my youth (under 19 years of age), in whole or in part, individually or in conjunction with other images and video footage, to be shared with the minimum number of individuals for the purposes of editing, compiling, and producing the video content for Richmond Delta Youth Orchestra’s virtual projects. I also authorize this completed footage to be displayed on RDYO’s website and other official social media channels, and to be used for media purposes including promotional presentations and marketing campaigns. Images may also be utilized for the RDYO website, concert posters, program brochures, and grant applications/reports.
Photos taken by an official RDYO photographer may also be shared with other parents for personal use only.

I waive rights to privacy and compensation, which I may have in connection with such use of my youth’s name and likeness, including rights to be written copy that may be created in connection with video production, editing and promotion therewith.

I am over 19 years-of-age and the parent or legal guardian of the youth, and I have read this waiver and am familiar with its content.
Adult Media Consent
