Disclaimer: Mending Hearts & Minds has made the decision to be an “out of network” provider with insurance companies. We can file out of network claims for you as a courtesy for Tricare, BlueCross Blue Shield, Cigna, Optum and UMR. We can also provide you with a superbill ( a receipt that contains all of the information your insurance company needs to process a claim) that you can use to file with your insurance company. When you use your insurance out of network, you are responsible for the fee at the time of service, we will file the claim )(or provide you with a Superbill), and any reimbursement by the insurance company will be sent directly to you. Out of network benefits can be very different than in network coverage though, so I would encourage you to call your insurance company to ask about your benefits prior to your first appointment.
